Odd bug leaving bear form. Add-on causing it?

Zag24Hi all. Thanks in advance for any help.
When I exit bear form in combat, my action bar is (often/always) left on the bear action bar. Of course, all my heals are on the humanoid action bar, so I go to heal and I just get an error that says I need to be in bear form for this action. It only happens in combat, it never seems to happen leaving cat form, and I can't say for certain that it happens every time, but it is more than half. I've already died a couple of times because the heal I was counting on never came.
This has only started happening with the last patch -- 2.2.3. The only mods I use that has anything to do with the action bar are CT_Mod and BindPad, though I do use quite a few others.
